Sub: Modification of Instructions regarding Air Travel Entitlements of Railway officers on Tour/ Duty and Delegation of Powers w.r.t. Sanctioning of Air Travel.
 
Ref: Railway Board’s letters No. F(E)1/2017/AL-28/41, dated 24.04.2018, 08.05.2018 & 10.05.2018 and Board’s letter No. F(E)I/2021/AL-28/47, dated 13.08.2021.
 
Instructions regarding air travel entitlements of Railway officers on tour/duty and delegation of powers w.r.t. sanctioning of air travel have been issued from time to time. It has now been decided in modification to all the earlier instructions on the subject that the revised instructions regarding air travel on tour / duty will be as following :
 
I. PHOD/CHOD in Zonal Railways/PUs, CAO/C in Construction Organizations, DGs in CTIs and DRM in Divisional offices may, for officers under their control, sanction travel by air on duty:
 
i. Without Finance concurrence for officers in Pay Matrix Level-14 & above
ii. With Finance concurrence for officers in Pay Matrix Level-13 & below.
 
II. In Railway Board, power to sanction air travel on duty by officers shall remain with AMs/PEDs. However, prior Finance concurrence shall be mandatory for air travel on duty by officers below the Director level.
 
III. Air travel entitlement for officers of Level-13 and below would be in Economy Class and for officers in Level-14 and above would be as per MoF/DoE’s instructions issued vide O.M. No. 19030/1/2017/-E.IV, dated 13.07.2017, i.e., in Business / Club Class.
 
IV. The officer will have to submit a self-certification in his/her Tour Programme and also with TA/DA claim that no Group C staff is being booked/has travelled on duty with the Officer.
 
V. If the reimbursement is being claimed, the same would be limited to actual air fare or three times of train fare of entitled class, whichever is lower.
 
VI. No extra Budgetary allotment would be provided.

विषय: बेंचमार्क विकलांग अभ्यर्थी के लिए पदोन्नति में आरक्षण के संबंध में स्पष्टीकरण।

आपका ध्यान रेलवे बोर्ड के दिनांक 01.07.2022 के समसंख्यक पत्र सं. (आरबीई सं. 74/2022) के साथ कार्मिक एवं प्रशिक्षण विभाग के दिनांक 17.05.2022 के का.ज्ञा. सं. 36012/1/2020-स्था. (आरक्षण-II) की ओर आकृष्ट किया जाता है, जिसके द्वारा बेंचमार्क विकलांग अभ्यर्थियों के लिए पदोन्नति में आरक्षण देने के लिए विस्तृत दिशानिर्देश जारी किए गए थे।

पदोन्नति संबंधी परीक्षाओं में बेंचमार्क विकलांग अभ्यर्थियों के लिए चयन/योग्यता के मानकों में छूट बढ़ाने के संबंध में अभ्यावेदन प्राप्त किए गए हैं। इस संबंध में आपका ध्यान कार्मिक एवं प्रशिक्षण विभाग के दिनांक 17.05.2022 के पूर्वोक्त निर्देशों के पैरा 8.1 एवं 8.2 की ओर आकृष्ट किया जाता है जो चयन में मानकों पर छूट देते है।

उपरोक्त उपबंधों में यह निर्धारित किया गया है कि मानकों पर छूट केवल तभी प्रदान की जाएगी जब निर्धारित मानकों के आधार पर पर्याप्त बेंचमार्क विकलांग अभ्यर्थी उपलब्ध नहीं हो। बहरहाल, दी जाने वाली अपेक्षित छूट की सीमा निर्देशों में इंगित नहीं की गई हैं।

मामले की जांच कार्मिक एवं प्रशिक्षण विभाग के परामर्श से की गई है एवं इस संबंध में स्पष्टीकरण निम्नानुसार है

“यदि अनुसूचित जाति/अनुसूचित जनजाति कोटि से संबंधित उम्मीदवारों के संबंध में विशिष्ट सीमित विभागीय परीक्षा/विभागीय परीक्षा के लिए किसी प्रकार की छूट की सीमा का निर्णय लिया गया है, जहां अनुसूचित जाति/अनुसूचित जनजाति कोटियों से संबंधित उम्मीदवारों की पर्याप्त सं. (इन कोटियों के लिए निर्धारित रिक्तियों के अनुएसार) सामान्य प्रक्रिया में परीक्षा उत्तीर्ण नहीं करती है, तो उन मामलों में छूट की सीमा ऐसे बेंचमार्क विकलांग अभ्यर्थी कोटि से संबंधित उम्मीदवारों के लिए बढ़ाई जा सकती है, जो अनुसूचित जाति/अनुसूचित जनजाति कोटि से संबंधित नहीं है। अगर बेंचमार्क विकलांग अभ्यर्थी कोटि का उम्मीदवार अनुसूचित जाति/अनुसूचित जनजाति कोटि से भी है, तब वे या तो अनुसूचित जाति/अनुसूचित जनजाति कोटि या तो बेंचमार्क विकलांग अभ्यर्थी कोटि के उम्मीदवार के रूप में विभागीय परीक्षा के लिए मानक की छूट दी जाएगी न की दोनों में।”

Sub: Reservation in Promotion to Persons with Benchmark Disabilities (PwBD).- Clarifications regarding.

Attention is invited to Board’s letter of even number dated 01.07.2022 (RBE . No.74/2022) along with DoP&T’s O.M. No. 36012/1/2020-Estt.(Res.-II dated 17.05.2022 whereby detailed guidelines were issued for providing reservation in promotions to Persons with Benchmark Disabilities.

2. Representations have been received on the subject of extent of relaxation of standards of selection/suitability for PwBD candidates in the promotional exams. Attention in this regard is drawn to Para 8.1 and 8.2 of DoP&T’s instructions dated 17.05.2022 ibid which provides for relaxation of standards in selections.

3. It is stipulated in the aforesaid provisions that relaxed standards are to be adopted only in case sufficient PwBD candidates are not available on the basis of prescribed standards. However, the extent of relaxation required to be given has not been indicated in the instructions. The matter has been examined in consultation with DoP&T and clarification in this regard is as under:-

“If any extent of relaxation has been decided for a particular Limited Departmental Examination/Departmental Examination in respect of candidates belonging to SC/ST category, where sufficient number of candidates belonging to SC/ST categories (as per vacancies earmarked for these categories) do not qualify the examination in the normal course, same extent of relaxation can be extended to candidates belonging to those PWBD category, who do not belong to SC/ST category. In case the candidates belonging to PwBD category also belong to SC/ST category, they would be eligible to be considered either as a SC/ST category or PwBD candidate for seeking relaxation of standard for Departmental Examination but not both.”

Regarding Admissibility of training allowance to teaching faculty

Sub: Admissibility of training allowance to teaching faculty of Centre of Excellence(CoE) for Modern Signalling (Kavach), IRISET, Secunderabad.

Ref: IRISET’s letter No. IRISET/ CoE/ Kavach/ Misc dtd 18.04.2023.

The proposal of IRISET regarding admissibility of training allowance @ 24% for Centre of Excellence (CoE) for Modern Signalling (Kavach) at par with CTIs faculty has been considered and the competent authority in Railway Board has agreed that the CoE, being a part of IRISET, its teaching faculty are eligible for training allowance @24% of basic pay subject to Board’s instructions on the subject matter issued from time to time.

2. The said allowance will be admissible to the teaching faculty only, who are involved in imparting training on regular basis and such posts need to be identified.

3. It must be ensured that instructions as per Board’s letter No. (MPP)/2013/3/18 dated 20.3.2015 (RBE no. 24/2015) regarding admissibility of allowance in each individual case by a committee constituted as per RBE No. 127/2001 should be followed. Also instructions as per Board’s letter no. E(MPP)99/19/1Pt dated 11.3.2003 (RBE no. 49/2003) and letter no. E(MPP)2012/3/28 dt. 06.10.2017 must be followed.

4. This issues with the concurrence of Finance Directorate of the Ministry of Railways.

Ref: ECoR’s letter No(s): (1) ECoR/Pers/Bills(NG)/Pay Protection/AK/CLA, dt. 11.11.2022. (2) ECoR/Pers/Bills (NG)/Pay Protection/JPS/Jr.Clk, dt. 04/05.01.2022 (corrected as 04/05.01.2023).

The matter referred vide ECoR’s letters under reference has been examined in Board’s office in consultation with DoPT who advised that as per the guidelines on fixation of pay of State Government employees on appointment to the posts under administrative control of Ministry of Railways issued vide Board’s letter No. E(P&A)LI/2016/PP-1, dt. 23.06.2016 (RBE No.73/2016) and No. E(P&A)II/2023/PP-3 dt. 11.05.2023 (RBE No.69/2023) pay protection is admissible in cases where a State Government employee is appointed in Central Government in identical or higher Grade Pay/Level. However, these guidelines do not contain any provision for pay protection in cases where a State Government employee is appointed in Central Government in a lower Grade Pay/Level. This infers that pay protection is not admissible in cases where a State Government employee is appointed in Central Government in lower Grade Pay/Level

Clarification Regarding Media Reports on recruitment for 9000 posts for Constables & Sub-Inspectors in RPF

मीडिया में रेलवे सुरक्षा बल (आरपीएफ) में कांस्टेबलों और सब-इंस्पेक्टरों के 9000 पदों पर भर्ती के संबंध में एक काल्पनिक संदेश प्रसारित किया जा रहा है। सूचित किया जाता है कि आरपीएफ या रेल मंत्रालय द्वारा अपनी आधिकारिक वेबसाइटों या किसी प्रिंट या इलेक्ट्रॉनिक मीडिया के माध्यम से ऐसी कोई अधिसूचना जारी नहीं की है।

A fictitious message is being circulated in media regarding recruitment for 9000 posts of Constables and Sub-Inspectors in Railway Protection Force (RPF). It is hereby informed that no such notification has been issued by RPF or Ministry of Railways on their official websites or through any print or electronic media.

Sub - Upper age limit for appearing Departmental selection for the post of Publicity Inspector in GP Rs.4200/Level-5.

Attention is invited to Board's letter of even number dated 14.11.2022 (RBE No. 151/2022), wherein mentioning instructions issued vide RBE No. 89/1989, it had been clarified that Level-I employees (erstwhile Group 'D'), who are now classified as Group 'C, are also eligible for the selection to the post of Publicity Inspector in Level-5.

One of the Zonal Railways has raised doubt as to whether the upper age limit along with relaxation for SC/ST employees as indicated vide RBE No. 131/97 still holds good or otherwise. The matter has been examined , and it is clarified that upper age limit of eligible employees for selection to the post of Publicity Inspector in Level-5 is 40 years which is relaxable by 5 years in the case of SC/ST employees as provided in RBE No. 131/97, which still holds good.

Clarification On Study Leave Rules by Railway Board

In terms of Rule 14 of the Study Leave Rules contained in Appendix -V of the Indian Railway Establishment Code (IREC) Vol-I(1985-Edition), if a Railway servant resigns or retires from service or otherwise quits service without returning to duty after a period of study leave or within a period of three years (eight years in the case of Railway Medical Service Officer who has been granted thirty six months study leave under sub-rule (2) of rule (2)) after such return to duty, he shall be required to refund;

i. the actual amount of leave salary, cost of fees, traveling and other expenses, if any, incurred by the Railways; and

(ii) the actual amount , if any , of the cost incurred by other agencies such as foreign Governments, Foundations and Trusts in connection with the course of study, together with interest thereon at rates for the time being in force on Government loans, from the date of demand, from his resignation is accepted or permission, to retire is granted or his quitting service otherwise.

2. In this regard, references have been received from various Railways seeking the details of the "Government Loan", the rate of interest of which is to be considered for charging the interest on the actual amount to be refunded by the Railway servant. Since DoP&T is the nodal Department on CCS(Leave) Rules, the matter was referred to them, who in turn asked M/o Railways to consult D/o Expenditure(M/o Finance).

3. After such due consultation, Board (MF & CRB) have considered the matter and decided as follows :

(i) The 'Advances' admissible to Government employees will be taken as 'Government Loan' as provided in Rule 14 of the Study Leave Rules contained in Appendix-V of IREC Vol I. and

(ii) The highest rate of interest prevailing on specific Advance at the relevant time will be taken as the 'rate of interest' for calculating the dues recoverable under Rule 14 of the Study Leave Rules.

Frequently asked Question (FAQ) On Privilege ePass / PTO

1. Q. What is the objective of privilege pass/PTO module ?

Ans. The objective is to enable serving railways staff to book tickets using E-pass/PTO which can be applied online and is issued online.

 

2. Q. What is the benefit of E-pass/E-PTO?

 

Ans. For E-pass / E-PTO staff does not need to go to pass clerk to apply for pass/PTO. They can apply from the comfort of their homes and get the E-pass / E-PTO.  Also, they can book tickets both using conventional means i.e. through the booking counter, And through IRCTC website or using UTS/PRS apps.

In addition details of the travelling staff and all his family members are verified from HRMS at the time of ticket booking using E-privilege pass/E-PTO .

 

3. Q. My name/ my family member’s name is longer than 16 digits, PRS accepts only 16 digits .How to enter my name/ family member’s names?

 

Ans. If name is longer than 16 digits, plz enter first 16 digits exactly as per HRMS name (mentioned on pass also) including the spaces only. PRS has constraint of 16 digits as it has to be printed on Tickets and they have limited space. For example if your name is Amaresh Kumar Srivastava which is greater than !6 digits plz use - Amaresh Kumar Sr. This is 16 digits including spaces. Plz don’t use A K srivastava or A.K. Srivastava or Amaresh kumar as it wont match with HRMS name which is validated during ticket booking. Please do not use punctuation marks (.) etc if they are not there in HRMS name.

 

4. Q. What is the validity of OTP for pass?

 

Ans. Presently the validity is 1 hour/ 60 minutes.

 

5. Q. I obtained OTP for pass, but the queue at counter was very long and my OTP expired, what to do?

 

Ans. OTP can be obtained again, if the earlier one expires. Also as explained earlier with E-pass/E-PTO you don’t even need to go to the counter, you can also book tickets using the IRCTC site. OTP can be obtained using your smartphone browser also from the HRMS site.

 

6. Q. Why has OTP been kept, when unique pass number is already there?

 

Ans. Since OTP will be sent on your registered mobile number and others should not have access to it, it has been kept as an additional layer of security so one that one does not misuse your E-pass / E-Pto.

 

7. Q. I am getting error during booking ticket booking on IRCTC site/ booking counter using E-pass/E-PTO? What to do?

 

Ans. Error is shown in such cases. During IRCTC booking its shown in red on top right of screen and for counter booking it is shown as bottom row on booking screen. The error mentions the reason why ticket booking failed. By understanding it , usually corrective action can be taken at your end. If it cant be resolved plz approach HRMS coordinator who will contact HRMS helpdesk to solve your issues.

Some sample errors with reasons are given below

 

a. Passenger details not matched or no more dependent- This issue arises because either of name, age or gender does not match with pass details. Also if pass is issued and family declaration is resubmitted by employee after that, this issue may arise. -

Most probable reason is that you have not typed passenger names as they are in HRMS, Please type first 16 digits including spaces, as they are in HRMS, as explained in point 3 above. Plz do shorten names using initials only. For example if name in HRMS is Roopesh Kumar Upadhyay plz don’t use R K Upadhyay , use Roopesh Kumar Up ( 1st 16 digits including spaces) , and if it is R K Upadhyay in HRMS plz don’t use Roopesh Kumar Up, use R K Upadhyay only.

 

One more reason for this error may be that one of your sons has crossed 21 years of age and you have not applied for extension of validity of his pass in HRMS family along with proper certificate and it has not been accepted. Sons after 21 years are allowed on grounds of- being bonafide student, being engaged in research work, working as articled clerk under CA or being invalid with appropriate doctor certificate. If you want the son to be included you have to first get your family member added in HRMS family and then give pass family declaration again. For applying for family members details modification and adding these family member certificates you can use ESS module preferably or you can approach your Service record/ bill clerk.

 

b. Essential integrity validation checks failed- Wrong data has been entered. For example you have applied for a class in a type of train for which you may not be entitled. For example, the train may be special type train classified as Rajdhani type train by PRS , and passenger may be trying for his entitled berths as per mail/express type train thinking train is mail/express or he may be entering wrong/expired OTP.

 

c. Return journey completed- Either return journey completed OR ticket cancelled by PRS/IRCTC but details not sent to HRMS by PRS/IRCTC , plz contact your HRMS coordinator to get ticket cancelled by HRMS.

 

d. Break journey station not in route- It means you have saved Via/break journey in pass that does not fall on route of train. Plz try to minimize number of via journey stations in a pass. Insert Via/break journey in pass, only when train that you intend to book ticket on, is passing through those stations.

 

e. Passenger entitlement is less than passenger travelling- It means you have applied for more than entitlement, I.e. more berths than entitled or higher class than entitled in that type of train. Plz ask PRS counter about the type of train and check your entitlements.

 

f. Check your journey date- Inward journey should be after outward journey date.

 

g. No ent in SUP train type for CC berth class- The train is ordinary mail/exp type train in which there is no entitlement of CC class which is there only in Shatabdi type trains. Plz refer RBE 16/19 for entitlement and check in which class which berths are allowed.

 

h. Other errors are also usually self explanatory. In case of doubt , helpdesk may be approached

 

8. Q. Why family has to be entered and accepted in HRMS first, why cant family declaration be given to pass clerk directly?

 

Ans. Family in HRMS is not only for pass purposes. It is planned to be used in future for other purposes also like welfare( SBF aid, scholarships), CGA appointments, CEA etc.

So HRMS should have complete correct family details. That’s why this process. After once family is accepted in HRMS, it is auto fetched for pass declaration and only family members eligible for pass family have to be selected by employee for giving declaration online.

 

9. Q. There have been changes in my family composition. (i.e. new child has been born, family member has died) what to do?

 

Ans. You have to get your HRMS family updated which can be done through ESS and update your family declaration adding or deleting the family member as per need.

 

10. Q. I was entitled for first class pass, I have now gone on Own request transfer to other unit in lower grade / have been medically decategorised and am entitled to first class pass. How to get first class pass issued?

 

Ans. In such cases employee has to himself go inside pass declaration menu, and enclose relevant document regarding own request transfer/medical decategorisation and submit it to pass clerk for verification, who will forward it to PIA. Once accepted he/she will get first class pass.

 

11. Q. I was appointed before 30.6. 87 and at that time I gave written option not to apply for widow pass. How will I get 6 set PTO instead of 4?

 

Ans. In such cases employee has to himself go inside pass declaration menu, and enclose relevant document regarding the written option and submit it to pass clerk for verification, who will forward it to PIA. Once accepted he/she will get first class pass.

 

12. Q. I have taken a PTO. I am unable to apply for break journey tickets on it?

 

Ans. PRS is not allowing cluster booking, group journey at present. So it will not be possible.

 

13. Q. I want to cancel ticket booked on PTO and book fresh ticket. How to Do it ?

 

Ans. Cancellation of ticket booked on PTO and rebooking is not allowed. Modification can be done only on tickets booked from counter and modification can be done by counter clerk directly only. This functionality is not available on IRCTC.

 

14. Q. I want to cancel my Privilege pass, how to do it?

 

Ans. Provision is there to cancel privilege pass. But pass is usually not cancelled ,except in exceptional circumstances. For cancellation of pass you have to supply documentary evidence regarding the exceptional circumstance like natural calamity, non sanction of leave etc. Cancellation and rebooking on privilege pass , on the other hand, can be done easily as per rules .

15. Q. I am working in pay level 1-4, I am entitled for one set IInd class “A” upgraded pass per year. How to avail it?

Clarification - Misuse of Complimentary Card Passes (CCPs)

Sub: Misuse of Complimentary Card Passes (CCPs)— clarification reg.

Please refer to Board's letter of even number dated 02.02.2022 (copy enclosed) vide which blank Complimentary Card Passes for various categories of non-Railway persons were distributed to all Zonal Railways. In addition to the supply of total 425 blank CCPs for various categories, instructions were also issued regarding nomination of Northern Railway as the Nodal Railway for getting the card passes printing in future as and when required by various Zonal Railways.

It is further mentioned that cases have come to light where misuse of Complimentary Card Passes has occurred. Based on the observations made by the Vigilance Dte. of Railway Board and in order to make necessary improvements in the system, it has been decided that whenever a request is, received from non-Railway person for renewal/extension of Complimentary Card Passes, a fresh card may be issued in lieu of old/expired pass. The card pass issuing authorities should ensure tagging of all expired card passes in the system at the time of issuing fresh card pass to prevent misuse by the user. Further steps may be taken to conduct 100% inventory checks of all kinds of CCPs issued to non-Railway persons and to carry out suitable message/flags in the system wherever any misuse is suspected.

Action taken in this regard may please be advised to this office.

In the PNM-AIRF a demand was raised regarding issue of Instructions to the effect that physically handicapped Railway employees may be posted near their native place. The matter was discussed in the PNM-AIRF meeting held on 5.4.2003, when it was explained that instructions for posting of physically handicapped employees near their native place already exist vide this Ministry's letter No.E(NG)1/91/TR/13. dated 11.2.92. It was however agreed to reiterate the relevant instructions. First Aid -FAQ & Do's / Don'ts for Burns

Q. Should I use ice to cool the burn?
No, use water only. Ice may further damage the skin.
Q. Should I put a plaster over a burn to make sure it doesn't get infected?
One should not use any adhesive bandages as it’ll stick to the skin and may cause further damage. Instead the burns should be covered with cling film or a clean plastic bag which will help prevent infection.

Q. If clothes are stuck to the burn, should I try to remove them?
No. Remove any clothing or jewelry near the burned area, but don’t try to remove anything that’s stuck to the burn. This could cause more damage.
Do's for burns
  • Call an ambulance for any serious burns. Burns to children or the elderly, electrical or chemical burns as well as burns to the face or genital area, should be attended to immediately.
  • Apply CPR if the person is not breathing normally.
  • Try to remove clothes and jewellery (from the area that has been burned) only if it is not sticking to the burned area.
  • Hold the burned area under gently running water, for about 10 minutes to half an hour.
  • To prevent corneal damage (in the case of chemical burns to the eyes), immediately irrigate the eyes with water or a saline solution.
  • For second degree burns on the limbs – elevate the limbs higher than the heart.
  • To reduce shock as well as loss of body heat, place clean, dry, non-fluffy cloths lightly over the burn.
  • Cover the person with a cool, wet, lint-free cloth, while waiting for an ambulance or when transporting the person to hospital.
Don'ts for burns
  • Apply lotions, butter, grease or oil to burned area.
  • Use ice, as it may cause frostbite.

Sub: Clarification w.r.t. extension of validity of unused Passes. Ref: Board’s letter of even number dated 30.06.2021.

Representations and RTI applications are being received in Board’s office complaining that Pass Issuing Authorities (PIA) are not extending validity of partly used Passes in terms of Board’s above referred letter and are deprived from using the Pass for unused portion. In this regard unused portion on Pass is illustrated below:-

On Pass issued for journey from Stations A to D with break journeys at Stations B & C, journey was performed only from Station A to B. The unused portions in such case are from Stations B to C and C to D.

2. In this context, attention is invited to Para-2 of Board’s letter referred to above which categorically stipulates that extension of validity should be granted on all unused Passes/PTOs upto 30.09.2021. In E-Pass Module of HRMS, all E-Passes, including partly used ‘E-Passes have been revalidated in the system itself for booking of tickets including for unused portion, upto 30.09.2021. Considering the instructions issued vide Board’s letter ibid and also in order to follow uniformity with E-Passes, the validity of Physical Passes should also be extended similarly. Accordingly, PIAs may be advised to take-necessary action to revalidate the partly used Physical Passes also upto 30.09.2021, if not already. done.

First Aid - Heavy Bleeding

Q. What should one do if the patient is bleeding heavily?
Put pressure on the wound with whatever available to stop or slow down the flow of the blood. Call local emergency numbers or ask for some body help to get to hospital. Keep pressure on the wound until the help arrives.
Q. The person looks pale and feels cold and dizzy. What does this mean?
It means there isn’t enough blood flowing through the body. It can be life threatening because it can very quickly lead to other conditions, such as lack of oxygen in the body’s tissues, heart attack or organ damage. This physical response to an injury or illness is called shock.
If you suspect someone is going into shock, lie him down and lift his feet higher than the rest of his body. Such that the legs are higher than the heart in this position, which helps increase blood flow to their brain and heart.
Q. Should wound be washed?
For minor cuts and grazes, one can wash the wound to remove any dirt. Don't wash a wound that is bleeding heavily. If a wound that is bleeding heavily put under a tap, then all clotting agents will wash away and will bleed more.
Do's and Don'ts for heavy bleeding
Do’s for heavy bleeding 
  • Reassure victim that help is on the way
  • Call ambulance immediately
  • Check victim’s status regularly
  • Use direct pressure to stop bleeding
  • Check to see if victim’s airways are clear
  • If no pulse or respiration, start CPR
  • To prevent transmission of disease, use latex gloves
  • Raise head if bleeding in upper body areas
  • Raise feet if bleeding in lower body areas
Don'ts for heavy bleeding
  • Don’t move the patient if not required
  • Always suspect “spinal injury” (and don’t move the victim)
  • Don’t set fractures and breaks (simply immobilize the victim)
  • Use “direct” pressure to stop bleeding
  • Don’t remove items imbedded in the eye
  • Don’t use burn ointments
  • Call emergency as soon as possible

First Aid Guide - Snake Bite
Snake bite is prevalent in our community for generations, and we are not able to prevent it. Education of the common man is required from snake bite, as well as measures to be taken after the bite. Snake bite may occur at any time during lifetime. WHO (2004) provided recommendations to reduce death due to snake bite as per international norms. A primary recommendation, based on evidence based procedures, was to establish a single protocol for both first-aid and treatment, and is relevant in Indian context as well.
Remember, traditional therapy have no proven benefit in the treatment of snake bite. Do not waste time and send the patient to hospital at the earliest.
First- Aid treatment protocol:
First-aid currently recommended may be remembered by mnemonic ‘’ CARRY NO R.I.G.H.T’’
CARRY: Do not let victim to walk even for short distance. Transport by conveyance, especially when bite is in legs.
NO:
NO- Tourniquet.
NO- Cutting.
NO- Electrotherapy.
NO- Pressure immobilisation, nitric oxide donor (nitrogesic ointment/nitrate spray)
R.I.G.H.T:
R: Reassure patient, since 70% of all snake bites are from non-venomous species. Only 50% of bites by venomous type of snakes actually envenomate (poison with venom) victims.
I: Immobilise limb in a fashion similar to a fractured limb, in case of bites on the limb. A bandage or cloth is used to hold the splints. Do not apply pressure and ensure that blood supply is not blocked. Compression in the form of tight ligatures does not work and may be dangerous even.
GH: Get to Hospital immediately.
T: Tell any systemic symptoms that manifest on way to hospital.
Do not waste time in first aid management by traditional methods which may dangerously delay effective treatment.
Diagnosis phase:
Resuscitation and treatment of breathing problem is a priority. Life threatening injuries should also be taken care of.
Patient is monitored for any worsening of symptoms, chiefly breathing or pertaining to cardiovascular system.
Look for fang (poisonous tooth) marks in the area of bite.
Limbs are divided into compartments of muscles, blood vessels and nerves. Compartment syndrome is a rare complication. It is seen in excessively swollen limbs. Severe swelling may cut off blood circulation to a particular compartment.
I. Diagnosis phase general assessment:
Diagnosis phase general assessment depends upon the type of symptoms. Depending upon the species, clinical features may include:
  • Local pain/ tissue damage. Bite area may show signs such as painful and tender wound, swelling, or blister formation.
  • Ptosis (drooping of upper eyelids)/ neurological signs. Local or systemic effect of venom may stop action of breathing muscles, resulting in death in untreated cases. Victim may have vision problem or difficulty in speaking.
  • Haemostatic abnormality leading to localised or diffuse bleeding.
  • Muscle death: Venom from certain snakes may produce muscle death. Debris of protein from dead muscle cells may affect kidney, since it may not be filtered out.
  • Renal damage.
Response to neostigmine and anti- snake venom (ASV) may be studied.
II. Diagnosis phase investigations:
  • 20 minute Whole Blood Clotting Test (20WBCT): This is most reliable bedside test of coagulation. A few milli-liter of fresh venous blood is taken in small test tube of glass. It is left undisturbed at room temperature for 20 minutes. After 20 minutes, tilt the test tube to check coagulability. If the blood is still liquid, then the patient has incoagulable blood. The test is carried out every half an hour for three hours and then at hourly intervals. If incoagulable blood is discovered, then six hourly cycle of ASV is adopted.
Send blood and urine samples to the laboratory to look for any evidence of bleeding, muscle death and for assessment of kidney function.
Management:
Management should be carried out under medical supervision.
Snakes can continue to bite and inject venom with successive bites till the venom is exhausted. Prevent a second bite or attack on another victim. Therefore, do not try to catch snake as this may lead to further bites. Identify the snake if possible but not at the cost of additional bite.
Every snake bite victim should attend emergency department in a hospital.
I. General:
  • Pain: Snake bite often causes severe pain locally. This may be treated with pain killers such as paracetamol.
  • Tourniquets: Though tourniquets are not advised, many a times these are still used. When used by someone, sudden removal of tight tourniquets may lead to massive surge of venom leading to neurological paralysis or hypotension. Tourniquet removal should be done under medical supervision, more so when pulse distal to it is absent.
Patients with local necrosis may be given antibiotics and tetanus toxoid booster.
Remove any constricting item such as ring which may cut off blood flow in case of swelling of bite area.
In past, suction was applied to remove toxin. It is no longer recommended, since suction may further damage local tissue.
II. Anti- snake venom:
When indicated, start ASV with whatever dose is available in hand (pending availability of full dose). In India, only polyvalent ASV is available. It is effective only against four common varieties of snakes (king cobra, Russells viper, saw scaled viper and common krait). Bites by other species, depending upon the geography, require special measures. These species need to be identified first.
Criteria for ASV administration:
ASV is a costly and scarce item. It should be administered only when there are definite signs of envenomation. Only free flowing and unbound venom, in tissue fluid or bloodstream can be neutralised.
ASV carries the risk of anaphylactic shock and therefore, should not be used unnecessarily.
Indications of ASV:
I. Systemic envenoming
  • Evidence of coagulopathy: Visible spontaneous systemic bleeding or coagulopathy is detected by 20WBCT.
  • Evidence of neurotoxicity: Victim is having muscle paralysis, ptosis, external ophthalmoplegia or is unable to lift head.
  • Abnormalities of cardiovascular system: Victim has signs of hypotension, systemic shock, cardiac arrhythmia or has abnormal electrocardiogram.
  • Severe and persistent vomiting or pain abdomen.
II. Severe current local envenoming
  • Severe current local swelling: Severe current local swelling involving more than half of the bitten limb, in the absence of tourniquet. In cases of severe swelling after bites on digits (toes and especially fingers) from species causing necrosis.
  • Rapid extension of swelling: Rapid extension of swelling (e.g. beyond ankle or waist within few hours of bites on hands or feet). Swelling arising after elapse of long hours of bite is not a ground for giving ASV.
Purely local swelling, even if accompanied by bite mark/s from an apparently venomous snake, is not a ground for giving ASV.
ASV administration:
Clinical decision is very important and the dose of ASV required varies from case to case. Ten to thirty vials are usually required. All victims do not require 10 vials of ASV. However, starting with 10 vials ensures sufficient neutralising power against average amount of injected venom. It also ensures neutralisation during next 12 hours of any free flowing venom.
No ASV test dose is given, since it does not have predictive value in detecting anaphylactic or late serum sickness reactions. Rather, these may pre-sensitise the victim and may pose greater risk.
Two methods of administration are recommended
  • Intravenous “push” injection: Reconstituted freeze-dried anti-venom or neat liquid anti-venom is injected by slow intravenous injection (not more than 2 ml per minute).
  • Intravenous infusion: Reconstituted freeze-dried anti-venom or neat liquid anti-venom is diluted in about 5 ml of isotonic fluid (isotonic saline or 5% dextrose) per kg of body weight, and is infused at a constant rate over a period of about 30- 60 minutes.
Recommended initial doses of ASV:
  • Neurotoxic/ haemostatic cases: Neurotoxic/ haemostatic 10 vials of ASV are needed, and the same amount of ASV is given to adults, children and even pregnant women. Snakes inject the same amount of venom in adults and children.
ASV is administered over 30- 60 minutes at constant speed. Liquid or reconstituted ASV in isotonic saline or glucose without any diluent fluid in volume overload victims is given.
Local administration of ASV near the site of bite is ineffective, painful and in fact raises intra-compartmental pressure.  Particularly, it is not injected in digits.
How long anti-venom is expected to be effective after the bite:
Anti-venom should be given as soon as it is indicated. It may reverse systemic envenoming even when it has persisted for several days, and in case of haemostatic abnormalities, for two or more weeks. It is appropriate to give anti-venom as long as evidence of coagulopathy persists.
ASV reactions:
  • Anaphylaxis: Anaphylaxis is a life threatening emergency. Victim is monitored for features such as fever, chills, itching, urticaria, hypotension and bronchospasm. ASV should be discontinued in anaphylaxis. Adrenaline should always be at hand. An extra dose of adrenaline may be required in patients who do not respond to initial doses. Noradrenaline and nitroglycerine should be available to correct hypotension in elderly. Intravenous fluids may be required for any haemodynamic instability.
On recovery, ASV may be restarted slowly for 10- 15 minutes, keeping the patient under close observation. After that, normal flow is maintained.
  • Late serum sickness: Late serum sickness may be treated with oral steroids. Antihistaminics may provide additional symptomatic relief.
III. Neurotoxic envenomation:
  • Neostigmine: Neostigmine, an anticholinesterase, may reverse respiratory failure and neurotoxic symptoms by prolonging the life of acetylcholine.
IV. Anti-haemostatic repeat dose:
In case of anti-haemostatic envenomation, adopted ASV strategy is to keep six hour time period in which clotting time is repeated. Repeat ASV dose is given over one hour in case of persisting coagulation defect. Same cycle is repeated until coagulation is restored or the species of snake is identified against which polyvalent ASV is ineffective. Repeat dose may be ten vials of ASV similar in quantity to the first dose.
V. Haematotoxic repeat dose:
Normal guidelines are to administer ASV every six hours until coagulation is restored. What should be done when 30 vials have been exhausted and the coagulation abnormality persist. One study has shown that even up to 50 vials (500 ml) may be given for haematotoxic poisoning. Envenomation by certain species does not respond to ASV. Coagulopathy may persist for up to three weeks in those cases.
VI. Role of surgery:
  • Surgical debridement: Surgical debridement of necrotic tissue may be done.
  • Fasciotomy: Fasciotomy is required when intra-compartmental pressure is high enough to collapse blood vessels leading to ischaemia. The role of fasciotomy is questionable.
  • Multiple puncture technique: Multiple puncture technique with large bore needle may be done to reduce intra-compartmental pressure. Intra-compartmental pressure may be measured using saline manometers.
